Agency assistance
This form sends an assistance request to First State. It does not by itself open a claim with your insurance company.
Missing a policy number, photos or an estimate shouldn’t stop you. Send what you know and we’ll help with the rest.

After the first step
We confirm we’ve received your request and contact you during office hours to help you report to your insurer or follow up on an existing claim. Our confirmation is not an insurer claim.
Your insurer’s own process applies. It gives you a claim number when it records the claim and tells you how it will assess the loss.
Report to your insurer or ask First State for help. Keep any reference or claim number you receive.
Your insurer may ask for more details, photos, documents or an inspection before deciding on the claim.
The issued certificate goes to the recipients you chose. Check the details when it arrives.
Common questions
Still unsure? Call your nearest office and we’ll talk it through.
Yes. Use your insurer’s listed reporting options and hours. Many insurers take reports online or by phone outside business hours. You can also send First State an assistance request at any time, but we’ll respond when the office reopens.
You can still ask First State for help. Give us your contact details and what you know about the incident, and we’ll help you find your policy details.
No, you can request help without them. If it’s safe, take photos and keep records such as receipts, and send more evidence when your insurer asks for it.
No. It sends an assistance request to First State. Your insurer opens a claim when it acknowledges your report. Keep the claim number it gives you.
Use your insurer’s claim-status option in the finder above, or select “Help with an existing claim” and include your claim number if you have it. Please don’t report the same incident again as a new loss.
That depends on your policy and your insurer’s assessment, including any deductibles and limits. Reporting a loss doesn’t guarantee payment. Your agent or insurer can talk through your situation.
Ask about your policy’s reporting requirements and your circumstances. Some policies require prompt notice of any loss, so talk to us before deciding not to report it.
These usually have their own reporting steps and forms set by the insurer or state. Use your insurer’s claims contact, or call First State and we’ll point you to the right process. This general form doesn’t replace those forms.
