First State Insurance Agency Southwest
Need proof of insurance for a job, load, contract or client? Send First State the certificate holder’s details, the requirements you received and your deadline. We’ll review the request against your policy and contact you if anything else is needed.
For business policies arranged through First State. Not sure whether your policy is with us? Call an office before you start. A certificate request does not change your coverage.
When we review requests
| Worthington | Mon–Fri, 8:00 AM – 5:00 PM |
| Sioux Falls | Mon–Fri, 8:00 AM – 5:00 PM |
| Pipestone | Mon, 9:00 AM – 1:00 PM; Tue–Fri by appointment |
All times Central. You can send a request at any time; it is reviewed during office hours.
Before you start
Copy names and wording exactly from the holder’s instructions. If you don’t have something, choose “Not sure” in the form and attach what you received.
Required
The legal name on your policy, plus a policy number if you have it.
Required
Exactly as the holder gave it, including Inc., LLC or other suffix.
Required
The insurance section of the contract, the holder’s instructions or a sample certificate.
Required
Where the finished certificate should go: you, the holder or someone else.
Required
The real date and, if it’s today, the time you need it.
Proof for a general contractor, owner or project before work starts, often with special wording. Learn more about
contractors insurance.
Proof for a broker, shipper or lessor, sometimes the same day a load is booked. Learn more about commercial trucking insurance.
Proof for a landlord, client or vendor agreement, including vehicles used for work. Learn more about commercial auto insurance.
Deadlines
Call during office hours and quote your request reference.
507-376-3950 Worthington · Mon–Fri 8–5
605-274-2545 Sioux Falls · Mon–Fri 8–5
After you submit
You get a reference and the deadline you gave us. This is not the certificate.
We compare the holder’s requirements with your policy records, not with the amounts entered on the form.
If something is missing or can’t be confirmed, we contact you before anything is issued.
The issued certificate goes to the recipients you chose. Check the details when it arrives.
Common questions
Still unsure? Call your nearest office.
A document that summarizes your insurance for someone else, such as a general contractor, broker or landlord. It shows coverage information; it doesn’t create or expand coverage.
The person or organization asking for proof of insurance. Its legal name and address may be different from the jobsite or from the person who receives the email.
No. If the holder has asked to be an additional insured, answer “Yes” to that question and send the wording. Additional-insured status has to be supported by your policy.
Enter today’s date and choose “Yes” for a same-day request. We’ll review it urgently, but completion depends on the information and requirements. Call us if your deadline is close.
Give us your business’s legal name and attach or paste the holder’s requirements. Choose “Not sure” rather than guessing.
Answer the four special-requirement questions and attach or paste the wording. We’ll check it against your policy before anything is shown on a certificate.
Yes. Choose “Correct” or “Repeat” in stage 1 and give the earlier reference if you have it. Confirm the current holder, requirements, deadline and recipients, because old dates and instructions aren’t reused automatically.
The holder can contact First State, but we must verify the request and the authority to release your information first. Submitting the form doesn’t by itself authorize issuance.
